New database aims to improve emergency general surgery care and outcomes

Researchers at Wake Forest Baptist Medical Center in Winston-Salem, NC, have successfully created and implemented an emergency general surgery registry (EGSR) that will advance the science of acute surgical care by allowing surgeons to track and improve surgical patient outcomes, create performance metrics, conduct valid research and ensure quality care for all emergency general surgery (EGS) patients. The registry, featured in a study published in the February 2012 issue of the Journal of the American College of Surgeons, was modeled after the American College of Surgeons (ACS) National Trauma Data Bank (NTDB) and components of the ACS National Surgical Quality Improvement Program (ACS NSQIP). It is the first registry of its kind to establish ICD-9 codes (International Classification of Diseases) that help to define and evaluate EGS patients.

An easier way to remove gallstones

For more than 100 years, the traditional treatment for the painful growths called gallstones has been removal of the gallbladder, or cholecystectomy. But a new device, patented in China, promises to make removing the entire organ unnecessary. A group of scientists from the Second People's Hospital of Panyu District and Central South University in China have developed an endoscope specially designed for locating and clearing out gallstones and other gallbladder lesions.

Robotic surgery with one small incision

On Tuesday (Dec. 20), Dr. Santiago Horgan, chief of minimally invasive surgery at UC San Diego Health System, was the first surgeon in the United States to remove a diseased gallbladder through a patient’s belly button with the aid of a new FDA-approved da Vinci Si Surgical System. With one incision, Horgan removed the gallbladder in 60 minutes. The patient returned home five hours after the groundbreaking surgery and reported minimal pain.

Milk thistle extract stops lung cancer in mice

Tissue with wound-like conditions allows tumors to grow and spread. In mouse lung cancer cells, treatment with silibinin, a major component of milk thistle, removed the molecular billboards that signal these wound-like conditions and so stopped the spread of these lung cancers, according to a recent study published in the journal Molecular Carcinogenesis.

Cortisol controls recycling of bile acids

Nature sees to it that we do not have "too much choler" (bile) in our body. A delicately equilibrated regulation system ensures that there is always exactly the right amount of bile in the gallbladder. When we are hungry, our body releases a hormone called cortisol, which is a glucocorticoid. Hepatic cells receive this hormone signal through their cortisol receptors (glucocorticoid receptors) and respond by filling the gallbladder with bile in preparation of the imminent food intake. Directly upon eating a meal, bile is secreted into the intestine.
