Under pressure from Medicare, hospitals hold more seniors for observation

Nobody wins when patients stay in the hospital unnecessarily, so the federal government in recent years has pushed hospitals to be careful about admitting Medicare recipients as inpatients. The apparent result is that more patients are being "held for observation" instead, according to a new study by Brown University gerontologists. While the shift in how hospitals care for elderly patients in the emergency department may reduce costs to Medicare, it can also increase out-of-pocket expenditures for patients.

ED chest pain units and physician discretion may lower stress test use

Rhode Island Hospital physicians report that managing chest pain patients within an emergency department chest pain unit by both emergency medicine staff and cardiologists is safe and effective and may lower the use of stress testing. A new study indicates that when patients were jointly managed and when stress testing was largely at the discretion of a cardiologist, stress testing use was lower and there was a low rate of 30-day major cardiac events. The study is published online in advance of print in Clinical Pathways in Cardiology.

Doctors' decisions on initial hospital admissions may affect readmission rates

Researchers compared hospitalization rates and rehospitalization rates of patients admitted for heart attack and for heart failure. Heart attack admissions are considered non-discretionary, whereas, heart failure admissions are considered more discretionary. Hospitalization after heart attack is mandated in treatment guidelines, so physicians have little or no room for discretionary decisions.
