Financial incentives affect prostate cancer treatment patterns

According to a new study by researchers at the Perelman School of Medicine at the University of Pennsylvania, prostate cancer patients of urologists who own expensive radiation equipment are more likely to receive radiation treatment in lieu of surgery than patients treated by urologists without an ownership stake in the equipment. The study, now available online in the Journal of Urology, found that integrated prostate cancer centers (IPCCs), where urology and radiation oncology practices are combined, use expensive radiation-based treatments at higher rates than other forms of prostate cancer treatment, and reduce their use of prostatectomy (the surgical removal of all or part of the prostate gland).

Follow-up to REDUCE study shows low rate of prostate cancer diagnosis

The four-year REDUCE (REduction by DUtasteride of prostate Cancer Events) clinical study evaluated prostate cancer risk reduction in men taking dutasteride, a 5-alpha-reductase inhibitor (5ARI) typically used to treat enlarged prostate. REDUCE results showed that dutasteride decreased the risk of biopsy detectable prostate cancer by 22.8 percent compared to a placebo group, but concerns remained about the drug's effectiveness. Results from a follow-up study are now published in The Journal of Urology.

Robotic-assisted radical bladder surgery potentially benefits bladder cancer patients

About 30 percent of the more than 70,000 bladder cancer cases expected in 2012 are muscle invasive. In such cases, radical cystectomy is the preferred treatment. In a pilot trial, a team of investigators assessed the efficacy of open radical cystectomy (ORC) vs. robotic-assisted laparoscopic radical cystectomy (RARC). While there were no significant differences in treatment outcomes, RARC resulted in decreased estimated blood loss and shorter hospital stay compared to ORC. The results are published in the February 2013 issue of The Journal of Urology.

MRI helps identify patients with prostate cancer who may benefit from active surveillance

PSA screening has resulted in improved prostate cancer survival, but the high rate of diagnosis and treatment side effects raise concerns about overtreatment. In the quest to prevent overtreatment, "active surveillance" has emerged as a plausible option, encouraged for men whose tumors may not need immediate treatment and may never progress to more serious illness. Appropriate criteria for selecting patients for active surveillance are continuously debated. A group of investigators from Memorial Sloan-Kettering Cancer Center in New York report that adding endorectal magnetic resonance imaging (MRI) to the initial clinical evaluation of men with clinically low prostate cancer risk helps assess eligibility for active surveillance. Their results are published in The Journal of Urology.
